建议你读读《理解“人为错误”现场指南》(The Field Guide to Understanding 'Human Error')。 https://www.amazon.com/Field-Guide-Understanding-Human-Error...
I suggest you read "The Field Guide to Understanding 'Human Error'". You'd learn a lot. https://www.amazon.com/Field-Guide-Understanding-Human-Error... My view is that expecting humans to stop making mistakes is much less effective than fixing the systems that amplify…
…superior, two feelings I have been known to enjoy. But it's a fundamentally bad way to approach analyzing safety issues. For those who really want to dig in on the topic, I strongly recommend Dekker's "A Field Guide to Understanding 'Human Error'": https://www.amazon.com/gp/product/B00Q8XCSFI/ref=dbs_a_def_r... It's nominally about examining airplane crashes. But he breaks down into great detail why the default analytical model is entirely inappropriate…
…I highly recommend: - For basic practices: Google's SRE chapters on incident management [2] - For the history of why we prepare for incidents and how we learn from them effectively: Sidney Dekker's Field Guide to Understanding Human Error [3] [1] https://kintaba.com [2] https://sre.google/sre-book/managing-incidents/ [3] https://www.amazon.com/Field-Guide-Understanding-Human-Error...
我快把《理解人为错误现场指南》(The Field Guide to Understanding Human Error)读完了:http://www.amazon.com/Field-Guide-Understanding-Human-Error/...
I'm almost done with The Field Guide to Understanding Human Error: http://www.amazon.com/Field-Guide-Understanding-Human-Error/... It's a brilliant book written by Sidney Dekker, a "Professor of Human Factors and Flight Safety". The basic point is that the default…
…Controlling and blaming are terrible for creative work, though. And they're also terrible for increasing safety beyond a certain pretty low level. (For those interested, I strongly recommend Sidney Dekker's "Field Guide to Understanding Human Error" [1], a great book on how to investigate airplane accidents, and how blame-focused approaches deeply harm real safety efforts.) So it's great to see Slack finding a way to scale up without losing something…
Blameless postmortems work phenomenally at Etsy which is a pretty low risk setting (after reading Sidney Dekker's book The Field Guide to Understanding Human Error [highly recommended] I would say that blameless post-mortems are even more important in a high-risk setting). Except failure isn't the correct word - the book makes the case that these are natural artefacts…
